TIMEFRAME TO REPORT DAMAGES

- 10 days from receipt of order
- If there is noticeable damage to any of the packaging, please take photos before unwrapping the items. Please keep packaging until items are inspected for damage. Please notate visible damage on delivery receipts.

The following items are required to file a damage claim (NO EXCEPTIONS):

Claim forms can be found in with your packing slip or you can have one emailed to you by submitting your request to us at claims@crestviewcollection.com.

RESELL CERTIFICATE    Download Form

** Please fill out the form in its entirety and email to claims@crestviewcollection.com or fax to 901-547-1129.

2) PICTURES

= ** REQUEST CANNOT BE PROCESSED WITHOUT PICTURES OF THE DAMAGED ITEM (S)**

1st Picture: A clear photo of either the product sticker, or the carton label.
2nd Picture: A clear photo of the overall piece.
3rd Picture: A clear close up photo of the damage or damages.

** YOU ARE MORE THAN WELCOME TO INCLUDE AS MANY PHOTOS AS YOU LIKE

** PLEASE SEND ALL OF THE ABOVE TO THE FOLLOWING:
